To register your team, enter the following information: 

For "Item Name", fill in your team's name.

For "Estimated Value", enter "0"

For "Item Description", enter "X"

Click the radio button if the team is associated with indivual(s) or if associated with a business. 

For the "remain anonymous" radio button, please select either "yes" or "no". 

For the contact information, please enter the Team's Leader information. 

When all information is entered, please press the "SUBMIT" button. 

Someone from the Community Cancer Network will contact you about signing up your team members and arranging for t-shirts.
